Abstract:
Background: The American healthcare system is projected to face a major RN shortage by 2030, with a nationwide shortage of over 500,000 RNs. High stress environments and lack of comfort contribute to 35-60% of new graduate RNs leaving their first place of employment. Among critical care RNs, the turnover rate in 2020 was 17% due to lack of confidence and discomfort. Post resuscitation debrief (PRD) attempts have been shown to reduce stress, improve teamwork, and provide medical education, but it is only performed in less than 25% of resuscitation events in most institutions. The purpose of this quality improvement project is to reduce stress among RNs in the Medical Intensive Care Unit (MICU) at the University of Alabama at Birmingham (UAB) Hospital through the implementation of an evidence-based PRD tool.
Framework: The project applies the Plan-Do-Study-Act (PDSA) Model for Improvement and will gather data utilizing a QR code for survey access and collection of demographic information, with any necessary changes made to the debriefing process following each survey step.
Methods: The PRD tool will be completed by the code leader after every code for a month, with a 5-minute debrief on the unit using the Rapid Post-Code Debrief tool. Participants will complete an anonymous postcode stress scale survey prior to the intervention, immediately following the intervention, and three months post-intervention. The Postcode Stress scale will measure stress scores pre- and post-intervention, and the results will be used to assess the success or failure of the debrief tool in reducing stress and analyzed using a paired T-test.

Implications/Sustainability: The post-code stress reduction from this project will decrease turnover in the MICU at UAB Hospital. The PRD tool can be incorporated system-wide to improve feelings of support for code members, reducing turnover at a larger scale.
